Limited License

Complete a limited license application if ALL of the following apply:

  • You have completed your program
  • You have not taken the NBCOT exam
  • You have had an occupational therapist complete the Direct Supervision Agreement and it is notarized

PRACTICE IS PROHIBITED BY LAW PRIOR TO ISSUANCE OF A LICENSE. APPLICANTS ARE URGED TO NOT ENTER INTO IRREVOCABLE CONTRACTS, PURCHASE, OR SALES AGREEMENTS ON THE ASSUMPTION THAT LICENSURE WILL BE GRANTED.

According to Arizona Administrative Code R4-43-205.C., the Board has 60 days to review applications for licensure.

Complete the steps below to apply for a limited license:

2 The occupational therapist assuming the professional and legal responsibility for your supervision must complete the Direct Supervision Agreement and have it notarized. Direct Supervision Agreement

4 Obtain professional recommendations from two health professionals. Recommendation Form
5 Complete your Statement of Citizenship form. (Arizona Revised Statutes § 41-1080 requires, in general, that a person applying for a license must submit documentation to the license agency that satisfactorily demonstrates the applicant’s presence in the United States is authorized under federal law.) 6 Scan the document that proves your citizenship status. List of Accepted Documents
7 Request verification of licensure from all states you have held a medical professional license, including those that are expired. You may use the forms provided by the other states, or our form. Verification must come DIRECTLY from the state. Verification of Licensure
8 Scan all of the completed forms into separate files. You will upload these documents to your online application.  
9 Complete the online application and pay your fees.  
10 You will receive an email from Board staff once your application is reviewed. This email will either say your application is complete or that documents are missing  
11 After your application is complete, the Board will review it at the next scheduled monthly Board meeting. If approved, your limited license will be mailed within five business days.

Take the NBCOT exam. Board staff will be notified when your exam has been scored.

  • Passing Score - Board staff will update your application. All you have to do is log in to the licensee portal and pay your remaining fee. 
  • Failing Score - Your limited license is immediately void.
